couple weeks ago two things came up on my notification bar. one is Star (white star with orange dot inside of it) and other is "+" (green) and they both giving me promos on new applications! i dont know how it got to my phone, maybe i downloaded bad app. i have Lookout app and scan shows nothing, no malware.

please help me remove it from my evo 3d.

It must be on of the applications you installed. It may not be easy to find out which one...just remove them one by one.
You can also try installing air push detector app. It has excellent reviews, and it may be an easier way find out the offending application.

Air push app helped. Bad app was "mp3 music download"
